How Does this Office Work?
The initial office visit is
approximately 1 hour long at which time
a thorough history will be taken to
review your symptoms, as well as a physical
exam will be done, and recommendations
will be made for any laboratory and
saliva testing needed to address your
concerns.
If saliva testing is done, and most
often it is needed, then a follow up
visit is required for a thorough
explanation of your results as it
relates to your symptoms and concerns.
Explanation of Fees
This office is a fee-for-service office.
This means you will pay for your office
visit at the time of service. We provide
the superbill (receipt) with all of the
necessary information needed to submit
to your insurance carrier for possible
reimbursement. We also have many of the
different insurance carrier forms needed
to send with your receipt.
Most major PPO insurance carriers will
reimburse a portion, if not a good
majority of your office visit back to
you. However this is subject to your
individual policy’s restrictions,
deductibles, and co-payments. We work
with laboratories that will bill your
insurance carriers for all labs done
through our office which then is subject
to your individual policy. If you have a
HMO type insurance this does not apply
and you will NOT be reimbursed for your
visit at this office.
Initial Office visit is very individual
but usually ranges from $200.00-$260.00.
This range varies based on the amount of
time spent with the patient and the
complexity of the care involved. Any
additional testing will incur an
additional charge.
Saliva Kits range from $82-$235.00 per
kit dependent on type of testing. Fees
are paid directly to saliva laboratory.
Follow-up consultation is required to go
over saliva test results with
recommendations at an additional charge
of $50.00 per 15 minutes.
Additional follow up visits range from
$95.00 and up dependent on time spent
and if any testing done.
